Added an o-qa action, which does not CC: or mentioned the maintainer.
This is useful when the Maintainer: has been set to the QA group
already.  Suggested by Pascal Hakim.

This form allows you to request diffs between any two revisions of this file. For each of the two "sides" of the diff, enter a numeric revision.
