Make the 0.5-1 version of the ganeti-instance-debootstrap

The patch also changes the build system from cdbs to dh, and updates the
policy to the latest, upgrade the debhelper compat level, etc.

Rename the etch instance directory

As upstream has started to generalize the instance and has renamed it, we also
change the name of the directory under trunk (ganeti-instance-debian-etch =>
instance-debootstrap). I've verified and the package still build correctly
after the change.
